The Patient Self Determination Act of 1991 brought much needed attention to the importance of advance care planning and surrogate decision-making. The purpose of this law is to ensure that a patient’s preferences for medical care are recognized and promoted, even if the patient loses decision-making capacity. In general, patients are presumed to have DMC. A patient’s DMC may come under question when distortions in thinking and understanding due to illness, delirium, depression or other psychiatric symptoms are identified or suspected. (...) Physicians and other healthcare professionals working in hospital settings where medical illness is frequently comorbid with depression, adjustment disorders, demoralization and suicidal ideation, can expect to encounter ethical tension when medically sick patients who are also depressed or suicidal request do not resuscitate orders. We show that the claimed mathematical equivalence of the models does not hold.

One of the major assumptions of John Zaller's RAS model of public opinion is that people need explicit cues from partisan elites in order to evaluate persuasive messages. This puts the public in the position of a passive audience, unable to scrutinize information or make independent decisions. However, there is evidence that people can, under some circumstances, evaluate and use information independently of elite cues. Thus, patterns of public opinion in the months before the Iraq war are inconsistent with the (...) predictions of Zaller's model. While the RAS model usually accounts for the dynamics of public opinion quite well, the situations in which it fails provide us with critical insights into the limits of elite influence. (shrink)

This article probes the relationship among constitutionalism, extra-legal prerogative power, and citizen judgment. While much has been written about the nature of Lockean prerogative, and while his theory serves as a direct inspiration for contemporary "normative extra-legalists," key participants in the debate over emergency powers, less attention has been paid to how the people judge prerogative. Attention to this issue is useful because an examination of the process of political judgment of extra-legalism in Locke leads to a complication of the (...) current extra-legalist vision of democratic mechanisms of accountability. The author argues that the extra-legal approach is right to consider the role of democratic publics in potentially constraining the exercise of emergency powers but wrong to formulate that role as one extra-constitutional power checking another extra-constitutional power. In the dyadic and triadic sharing of emotions, intentions, and behaviors in families, interactive synchrony is important to the early life experiences that contribute to the development of cultural cognition. This synchrony likely depends on neurobiological circuits, currently under study with brain imaging, that involve attention, stress response, and memory.

It is proved (Theorem 1) that if 0♯ exists, then any constructible forcing property which over L adds no reals, over V collapses an uncountable L-cardinal to cardinality ω. This improves a theorem of Foreman, Magidor, and Shelah. Also, a method for approximating this phenomenon generically is found (Theorem 2). The strategy is first to reduce the problem of `disabling' forcing properties to that of specializing certain trees in a weak sense.

ABSTRACTAnxiety and depression diagnoses are associated with suicidal thoughts and behaviours. However, a categorical understanding of these associations limits insight into identifying dimensional mechanisms of suicide risk. This study investigated anxious and depressive features through a lens of suicide risk, independent of diagnosis. Latent class analysis of 97 depression, anxiety, and suicidality-related items among 616 psychiatric outpatients indicated a 3-class solution, specifically: a higher suicide-risk class uniquely differentiated from both other classes by high reported levels of depression and anxious arousal; (...) a lower suicide-risk class that reported levels of anxiety sensitivity and generalised worry comparable to Class 1, but lower levels of depression and anxious arousal; and a low to non-suicidal class that reported relatively low levels across all depression and anxiety measures. It is shown that there is no satisfactory first-order characterization of those subsets of ω 2 that have closed unbounded subsets in ω 1 , ω 2 and GCH preserving outer models. These “anticharacterization” results generalize to subsets of successors of uncountable regular cardinals. Similar results are proved for trees of height and cardinality κ + and for partitions of [ κ + ] 2 , when κ is an infinite cardinal.

Achievement test differences between boys and girls and between young men and young women, mostly favoring males, extend far beyond mathematics. Such pervasive differences, illustrated here, may require an explanatory theory broader than Geary's.
